      Small BCCs with well-defined borders in low-risk locations such as the trunk, arms and legs are considered low-risk.

      Low-risk BCCs may be treated with excision, electrodesiccation and curettage (scraping and burning), or cryosurgery (freezing) with very high cure rates.

      Some superficial BCCs can be treated successfully with prescription creams or with photodynamic therapy (PDT), although there is a higher risk of incomplete treatment or recurrence.

      Treatment of High Risk Basal Cell Carcinoma

      BCCs at greater risk of recurrence after treatment are considered high-risk BCCs. These may arise in high-risk areas such as the head and neck, may be larger, may exhibit aggressive growth under the microscope, or may have ill-defined borders.

      Mohs micrographic surgery is often recommended for high risk BCCs to increase the probability of cure. This is a specialized technique performed in the outpatient setting under local anesthesia, and which offers the highest possible cure rates via precise, thorough evaluation of cancerous tissue margins while minimizing removal of healthy tissue.

      Radiation treatment is considered for properly selected BCCs when surgery isn't possible, is contraindicated, or if the patient prefers another option. Options include external beam radiation, superficial X-ray therapy, and brachytherapy. Dr. Ratner recommends that her patients see a Radiation Oncologist for consultation when this type of treatment is being considered.

      Newer oral treatments or immunotherapy may be considered for selected high-risk or locally advanced BCCs which may not be treatable with surgery or radiation. While Dr. Ratner prescribes oral treatments for advanced BCC, she refers her patients to Medical Oncology for immunotherapy or other targeted therapies.
